BY USING OUR SERVICES, YOU AGREE TO BE BOUND BY THE TERMS AND CONDITIONS OF THIS NOTICE AND TO OUR COLLECTION, USE, AND DISCLOSURE OF PERSONAL INFORMATION, INCLUDING PERSONALLY IDENTIFIABLE HEALTH INFORMATION (PHI). IF YOU DO NOT AGREE TO ANY TERM OF THIS NOTICE, DO NOT USE THE SERVICES AND EXIT IMMEDIATELY.
Scope of Notice
This Notice of Privacy Practices (“Notice”) explains the collection, use, and disclosure practices of services owned and operated by Stack Health LLC (“CoreLife”). This Notice constitutes an agreement between you and CoreLife while accessing and using our online or offline Services (the “Services”). The practices described in this Notice apply to information gathered through digital services offered by CoreLife, information you submit to us offline, and Personal Information (as described below) that we may collect from third parties.

Information Collected Directly from You
You can visit our services without disclosing any personal information. Any information you provide to us is voluntary, but some information may be necessary for us to provide certain services, offers, and content. CoreLife may ask for personal information such as your name, email address, phone number, zip code, and other identifiable information to provide you with information and referral services. You may also be asked to provide information about another person for whom you may be seeking care services, with their permission.
CoreLife does not collect biometrics or sensitive information unless necessary to provide the requested service. We may periodically supplement the Personal Information we collect from you with information from other businesses, business partners, contractors, and third parties.
Information Collected from Your Use of the Services
CoreLife may collect information about you using analytics tools or other technologies like cookies and web beacons. This includes the pages you view, the links you click, and other actions you take. We may also collect standard information such as your IP address, browser type, language, access times, and referring website addresses. We may supplement aggregate information with similar information obtained from companies with whom we have marketing or other business relationships.

  • For Legal and Compliance :  To comply with the law or protect rights and safety.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires that all protected health information and other identifiable health information used or disclosed by us in any form are kept confidential.
Your Rights
You have the right to manage, update, and delete your information by contacting us at support@Corelifemd.com. We will respond within a reasonable timeframe. If you delete necessary information, you may not be able to use certain services.

Patient Bill of Rights
  • 1. A patient has the right to respectful care given by competent personnel.

  • 4. A patient has the right to have his or her medical records treated as confidential and read only by people with a need to know. Information about a patient will be released only with permission from the patient or as required by law.
  • 5. A patient has the right to request amendments to and obtain information on disclosures of his or her health information, in accordance with law and regulation.
  • 6. A patient has the right to know what rules apply to his or her conduct as a patient, as stated in the Patient Responsibilities section below.

  • 8. A patient has the right to make informed decisions regarding his or her care and has the right to include family members in those
    decisions.
  • 9. A patient has the right to information from his or her provider in order to make informed decisions about his or her care. This means
    that patients will be given information about their diagnosis, prognosis, and different treatment choices. This information will be given in terms that the patient can understand.
  • 10. A patient has the right to refuse any drugs, treatment or procedures to the extent permitted by law after hearing the medical consequences of refusing the drug, treatment or procedure.
  • 11. A patient has the right to have help getting another provider’s opinion at his or her request and expense.
  • 12. A patient has the right to care without regard to race, color, religion, disability, sex, sexual orientation, national origin, or source of payment.
  • 13. A patient has the right to be given information in a manner that he or she can understand. A patient who does not speak English, or is hearing or speech impaired, has the right to an interpreter, when possible, and/or as provided by law.
  • 14. Upon request, a patient has the right to access all information contained in the patient’s medical records within a reasonable timeframe. This access may be restricted by the patient’s provider only for sound medical reasons. A patient has the right to have information in the medical record explained to him or her.
  • 15. A patient has the right to be free from needless duplication of medical and nursing procedures.
  • 16. A patient has the right to treatment that avoids unnecessary discomfort.
  • 17. A patient has the right to a copy of his or her bills. A patient also has the right to have the bill explained.
  • 18. A patient has the right to request help in finding ways to pay his or her medical bills.
  • 19. A patient has the right to access people or agencies to act on the patient’s behalf or to protect the patient’s right under law. A patient has the right to have protective services contacted when he or she or the patient’s family members are concerned about safety.
  • 20. A patient has the right to be informed of his or her rights at the earliest possible time in the course of his or her treatment.
  • 21. A patient has the right to make advance directives (such as a living will, health care power of attorney and advance instruction for mental health treatment) and to have those directives followed to the extent permitted by law.
  • 22. A patient has the right to personal privacy and to receive care in a safe and secure setting.
  • 23. A Medicare patient has the right to appeal decisions about his or her care to a local Medicare Review Board. The Facility will provide the name, address, and phone number of the local Medicare Review Board and information about filing an appeal.
  • 24. A patient has the right to be free from all forms of abuse or harassment.
  • 25. A patient has the right to be involved in resolving dilemmas about care decisions.
  • 26. A patient has the right to have his or her complaints about care resolved.
  • 27. A patient has the right to be free from financial exploitation by CoreLife.
Patient Responsibilities
  • Provide complete and accurate information concerning your present health, medication, allergies, etc., when appropriate to your care/service.
  • Be involved, as needed and as able, in the development, carrying out and modifying your care plan.
  • Properly clean and maintain equipment and supplies.
  • Contact us with any questions or problems concerning your equipment, supplies or service.
  • Notify us prior to changing your place of residence or your telephone number.
  • Notify us when encountering any problem with equipment or service.
  • Notify us of denial and/or restriction of our privacy policy.
